All Graduate School forms should adhere to these guidelines.
Use the official Graduate School logo (Style2) at the top of all forms. To insert the logo in a Word document:
All approved logos - Graduate School and UK - are located on the j: drive at j:\data\gs\LOGOS\. Here is a sample of both logos as well: Graduate School and UK Official Logos (pdf).
In the event that you need to use the UK logo without the Graduate School identification, you should use one of the approved UK logos.
For forms that have narrative as a first page (as several of the fellowship forms do), the logo should appear on the page that is the actual form.
In the case of a multiple-page form, the logo goes on the first page.
UK Public Relations has approved several common fonts for use on UK forms. Use the Arial font for Graduate School forms (this includes the variations of Arial such as Bold and Italic).
Always include the date the form was created in the lower left hand corner of the page in 8-point type, in mm/yy format (e.g., 10/03).
Copy the finalized form to the j: drive at j:\data\gsfellow\ for fellowship forms or j:\data\gs\GraduateSchoolForms\ for all other forms.
