OCCUPATIONAL HEALTH FOR ANIMAL WORKERS
EDUCATION AND PREVENTION

PHS Policy (IV,A,1,f) requires a health program for personnel who have contact with animals as well as personnel who work in laboratory animal facilities. IACUC Document 106: "Training and Continuing Education Requirements" specifies the requirements for the Occupational Health Program.

1. Occupational Health Training.  This training is provided by the Division of Laboratory Animal Resources as a pdf document downloaded from the DLAR website at www.research.uky.edu/dlar/links.html

2. Animal Worker Questionnaire.  Completed forms may be submitted to the ORI-IACUC in HSRB, Room 322 or faxed to 859-323-9882.  Forms may be downloaded from: http://www.research.uky.edu/ori/univet/resources/occupational_health/occhlth.doc

Note: The "Animal Worker Questionnaire" must be re-submitted every three years.
