Internal Approval Form and Disclosure of Financial Interest Form Revised

The Office of Sponsored Projects Administration has issued revised Internal Approval Form (IAF) and Disclosure of Financial Interest Form to replace social security number with UK Person ID. Instructions for the IAF have also been updated. Several other changes were made to the IAF, including:

• Clarifying that departments should be identified by department number rather than name
• Adding approval dates and numbers for the IBC and RSC
• Adding a resource question pertaining to use of the Clinical Research Development and Operations Center (CR-DOC, GCRC/UKCRO)
• Adding a certification required for proposals to the National Institutes of Health
• Adding separate designations for paper receipt deadlines and electronic receipt deadlines
